Bank of America, Chase bank, USPS - all said they can not notarize passport/visa related documents.
Then did in UPS - They charged $6 per document.
I got my documents notarized at Bank of America here in Upstate NY for free. Most BOA locations do notarize.
Regarding the photos, I had to submit only four ( 3 on national verification and one on form NO-1) she returned the remaining 4 at BLS New york when I was there to submit everything in person
The mail said Order Form Passport # but didn't have that attached. Guess it happens sometimes. I had to fill another order form and then I got a mail with both attachments!

I had submitted the passport- reissue (after 10 yrs) for my wife to SFO consulate a couple of weeks back. Thought of sharing a couple of things
Issue #1.I used Fedex return envelope and filled it as directed by the person at Fedex office and they had suggested that I open a fedex account(which i did) and put the account number on the return envelope that i send along with my passport and forms to BLS
I followed this but got a comment from BLS on the website that since there was 'no prepaid printed bar code, we cannot accept this return envelope'
So I had to send another envelope with a printed barcode - this time i did it from USPS and for around $19 i got the stamps on the envelope and sent it out.
Thought of sharing it with folks planning to use Fedex. Writing the account number alone apparently doesnt work for BLS International though Fedex insists its the right way
Issue 2. Another strange issue we are facing is that even though the status on BLS site says that documents were now accepted and sent to Consultate, we got a call from a person who said he was BLS employee and claimed that the Money Order we sent got stolen. He said this happened for a few money orders and we need to resend the money order. My wife had asked him to send a mail from BLS id and he did send a mail from bls international mail id asking for the same. It was very odd!
However when we called the call center they had no idea about this and said the passport and docs have been submitted to SFO consultate and they have no further updates.
I got mail address of SFO center and have mailed them to check if the email i got for sending money order again is genuine.
Did not get any response yet, pretty confused and really irritated with the way BLS is handling things

My passport got renewed. Now I have a question.
My visa is in old passport. I am travelling to India after 1 month.
I know I need to carry both passports while travelling.
But do I need to transfer my visa from old to new passport before coming back to US?

@Ajay, you were lucky to get it notarized at BOA. Some branches might not have permission. My bad luck.
There was a confusion in the checklist regarding photos. To be on safer side I sent 4 photos as mentioned in the checklist. They sent back only 1 photo that means they need 7 photos right? Anyways passport got renewed. :)

@Sudheesh if your names are same as in the new passport you don't have to do that and your visa and i-94 and dmv names are all same, you don't have to. for me i have FNU in my names i want to remove them.

Don't worry. You are well within the usual processing time ( 3-4 weeks). I applied for mine on December 3rd and got an email from BLS this Monday saying that my passport is ready for pick up

What can I do give me some advise
After mailing an application to SFO Got an email stating you have not provided Affidavit event though I sent it through application on 23 dec 2013. then called them on 24 th and they said that they have sent request for rechecking application and you will get Email within 48 hrs.called them again on 27th told me to wait till 30th dec for Email. On 30th Dec 2013 got my whole application back through mail. No status update on application no Email..Again called SFO center then raised some request again and told me to wait again for 48 hrs.This is awful :(

No. I applied in New York. I went in person and collected my passport on January 2nd. Passport was ready to be picked up on December 24th 2013. I submitted my application in person on December 2nd, 2013

Query regarding returned application from BLS
I applied for renewal of my passport at BLS SFO through mail.
However because of the unique-ness of my application (with name change etc.) the application got returned by BLS asking for some more documents. And i see that the Check that i sent with the application is not returned by BLS.
Now I have a question, when I send the application again with more documents, Am I supposed to send them another cheque for the same amount? Or just the processing fee? Or nothing at all?
I could not find this information anywhere?

I will be applying for my passport renewal in Chicago. Visa stamped on my passport expired in September 2013. I have my approved I-797 and new I-94 till 2016. Procedure to submit 7 extra copies of National verification is NA for Chicago. Does this mean that that I need to submit just 1 copy of National verification or visit the BLS office in person?

The Indian Address in my passport is not applicable anymore. Instead, I can give a different 'permanent' Indian Address. Might it make the whole-passport-renewal more complicated/time consuming for this change ? If anyone faced similar situation/has some experience and share - it would be great. Regards.
p.s. I have no problem about giving the details/supp Docs on my current address / US Address. Additionally I have to state the 'permanent Indian Address' and that is where my question is.

I am applying for renewal of my passport at SFO, I have filled all the docs and will be mailing it out today, I have two important questions,
1. I've filled the BLS order form and have paid for the prepaid shipping services and got a Pickup Shipping Label along with the other information, what I didn't get is the 'Return Shipping Label', are we supposed to get a return shipping label (as it's a prepaid service for pickup and delivery), or do we have to purchase a prepaid return shipping envelope and send it along with the passport package ?
2. I read in the above comments, that cashiers check to 'BLS International USA' has been returned back and they've asked to make it under 'BLS International', is this a common issue ? Currently, on the BLS order form it states to address the money order/cashiers check to 'BLS International USA Inc' and I have made it according to what it states currently.

Posting my experience below-.
1. I would suggest go to the BLS Passport website and try to get a basic understanding of the process. It is somewhat confusing, but the best possible option to get you started.
2. Before filing any forms / applications, i decided to get all my documents in order. You can check which documents you will need on the BLS website,
3.Get 2in X 2in photographs. I got mine from CVS. Just tell them you need a passport size 2inX2in photograph. I got 7 photos to be on the safer side.
4. Photocopies- 1st 5 and last 5 pages of the passport. They do not have to be notarized. Notarized photocopy of residential proof- I had my driving license photocopy which i notarized from Bank of America.
5. This is IMPORTANT!- My 1st H1-B visa had expired on September 30th, 2013. I had received the new I-797 approval notice for my extension. The BLS website states that if your US visa has expired, you need some additional documents to be sent to them. I had emailed BLS, and had got a reply stating as long as i have the valid I-797 notice, i am not required to send those additional documents. Make sure you take a COLOR photocopy of the I-797, notarize it.
6. To be on the safe side, i had filled up a Change of Appearance form. I stated that my appearance had not changed, pasted my photo, and had that form notarized as well.
7. I wanted to get the regular 36 page passport. I went to Bank of America, got a money order for it.
8. Check Sheet- You can find this on the BLS website. It just says what documents you have included. Make sure to Tick appropriate fields.-SIGN IT
9. Postal Application form- This is CONFUSING. The webpage on BLS website which lists all the different steps, documents required, does not say anything about this form. However, if you look under the HOW TO APPLY-PROCESS TO APPLY BY POSTAL link, it talks about this form. Better to fill it and send it.-SIGN IT.
10. Original Passport. DO NOT FORGET THIS AND TAKE YOUR I-94 OUT OF THE PASSPORT!
APPLICATION-
1. National Verification Form. It can be found on the BLS website. Save a copy, and then either fill the form and print it, or print it and manually fill it. You need 3 copies to be sent of this form. Each form will have 1 photo glued to it. ONLINE APPLICATION- BLS website will direct you to it. Some parts of the application are filled online. Then you save the form, print it, and fill the remainder of the form manually. Make sure ti fill up as much as possible. You will need to glue 1 photo on the application form (and sign at the end of the form. DO NOT FORGET TO SIGN).
2. I then went to get my envelopes in which i was going to send the documents.The BLS website states that the first thing you should do is fill out an order form. However, when you are filling it, it asks for tracking numbers of your envelopes .Hence please make sure you have those numbers ready before you fill the order form.
3. I got my envelopes from FEDEX. I went on FEDEX.com, created an account. Then i created a shipment to send my documents. I had a tracking number for that. I also created a return shipment, had a separate tracking number for that. Printed 2 shipping labels.
SENDING DOCUMENTS
1. This was my last step. I filled out the BLS order form. Once you submit the form, you get an email with a shipping label attached. Print it out. You need to attach this shipping label along with the FEDEX shipping label on the envelope in which you are sending the documents.
2. I also received a BLS order form copy. I took a printout, and attached it with the rest of my documents.DO NOT FORGET THIS.
3. Went to the FEDEX branch. I placed all the documents in the envelope. I would suggest get 1 big and 1 small envelope. Use the small envelope for BLS to return your passport. Place the small envelope and your documents in the 1st envelope. Make sure you attach the correct shipping label on each envelope.Then i sent it through next day Air (my preference).
This is just a general summary. I hope i haven't missed anything. And i hope it helps.

It should be the new name/surname that you want printed on passport. And you should provide valid proofs for the name change along with that.
@Adeeti : They do accept indian notarized document. I had sent my Dad's passport copy which was notarized in India and there was no problem. What documents you are asking about ?

Where to put the form attached with this email in the document stack? The checklist doesn't mention it, so I'm not sure of the order.
Also, does the first 5 pages of passport mean page numbers 1 through 5 or pages 1 through 10 (both visible sides considered as one page)?
Lastly, how many proof of address are required? Is driver's license alone sufficient or should I attach utility bill too?
Passport has page numbers. So take copy of pages 1 to 5. Similarly take copy of last 5 pages.
Dear unknown: The copy of the attachment should be kept just before the checklist. First keep the attachment, then checklist, then as per the checklist.
I sent only DL for address proof.

I drooped it of in person and had paid for the courier service.. I received mine in a Fedex..
it is shame that the delivery does not even require a signature.. they just drooped it @ my door step. so be careful if you use their service...

Since then status has not changed.Its been close to 4 weeks since I sent my passport. I tried calling BLS office many times, they will keep us in waiting for 30mins and call gets disconnected after that. I tried to mail them,escalate the issue. Some one just responds with very generic information like a machine "We have received your application. Please allow 3 weeks to process the application".
What I am not sure about is,
When does 3 week processing time starts? Is it when BLS receives the application or when BLS accepts the application.

BLS accepted application on 07 Apr 2014
Worried because I am on I-94 and need to apply for visa extension. and egarly waiting for renewal. Any inputs would help me.

Hoping one of you have the same case like me.
My passport is expiring next month June 2014.
I have valid B1 visa that I got few years back for 10 years and its valid until 2016.
I currently have Green Card which expired in Feb 2014 but I got I-797 Notice of action for I-751 application (removal of conditional permanent residency from USCIS) and it says, my conditional resident GC status has been extended for a period of next 1 year therefore until Feb 2015 (but I don't have GC reflecting the same, its just I-797 notice of action Form I have).
My question - Because I still have valid B1 visa until 2016, do I still fall in "No Status" condition and do I have to fill all 7 National Verification Forms + Passport and GC copies of two Indians who have Green Card here??

Though I dropped my passport renewal application in person at BLS NY, I thought I still should share my experience.
I am on my OPT with EXPIRED F1 visa (please be advised even with expired VISA, OPT gives you a status and one does not need a "no status affidavit")
General Experience: To start with, I would like to say may be BLS is not good in responding to phones/emails, they try their best at the service center to get your work done. I saw applicants there who had no clue even what a passport application looks like and they wanted BLS to help them from scratch, which of course BLS employees did (but, quite frustrating for other applicants, prepared and waiting).
I went from Boston, reached at BLS before 9 AM,though my appt was at 11:45 AM. Sadly, I had to wait till noon for to be called.
Photos: CVS, 4 total (3 NVF, 1 Online form). I carried 10 to be on safe side.
Notarized: I got all the copies
notarized, even my OPT, just in case.
Facial appearance change: Come on! face does change over 10 years. Though nothing significant changed with my face, I still filled and attached that form as well, you never know. Face change is very subjective.
Money: They take cash at the counters. I got USPS money order for 116.20 (Jumbo booklet).
Everything went well at the counter. These are my timelines:
Application accepted by BLS: 01-07-2014 , 12:31
Application Processed on Date :01-07-2014
Application Processed and submitted to Consulate : 07-07-2014 , 16:17

I received my renewed passport today (22 July 14) by UPS after a week long "passport dispatched to applicant" status update. I think BLS creates the shipping label, but takes 5-7 days to actually hand the packages over to UPS/USPS/FedEx. Be patient.

My wife needs to renew her passport. But we have a 30 days old baby. Can i go to the BLS office & file the application without her. I live close to houston so i can go there instead of mailing.
I did this 3-4 times and it never got rejected.
For photos you can use http://www.epasspartphoto.com. You can ask someone to take your photograph and then upload to the site and then adjust the photo as per rules, take a .jpg print and upload to print on glossy paper from either walgreen/walmart of CVS and it cost only 29 cent per 4photos. You can opt to postal option of website sending you photograph too which is still cheap.
If you take photos from CVS/Walgreen/Walmart you are at mercy of photographer at that location and equipment. Many of them are not trained. We tried in one of the store and they sometims dont know basics.

3)On Jan 7th 2015 my application has been accepted by BLS
4) Application processed and submitted to consulate by BLS on Jan 8th 2015.
5) Passport received to BLS center on Jan 16th 2015
6) I received it on Jan 19th 2015

You mentioned that we need to provide the US address and not the Indian address. My concern here is, that way I would lose the proof of address of Indian address. Is there any way I can have the Indian address printed on the new passport? My old passport (which is still valid for next 5 months) would serve as the address proof for that.
Also, instead of sending the docs via mail, if I personally visit the embassy and submit the docs, can I get the renewed passport in a day or two?

I am currently staying at the apartment on lease and do not want to put that address in my passport. Is there a way i can still use my India Address or an option that i can put a relatives address where i stayed before 1-2 years ?

I need your help. My passport expired on Apr 2013. i am confused about the Nationality Verification form. I know I need to submit 7 copies of my Nationality Verification form. When i got my documents notarized by the notary, I see the following text....." This instrument was acknowledged on ---, ----, 20---, by ----------------". In the by dash, the notary's name was placed. My best guess is that the name should be mine going by the wording on the notary seal. Does anyone have information on this as to how this should be filled?

Hello Thomas. It should be the name of notary. Notary is comparing the copy of the document with original and certifying that he/she verified the original and the copy is a true copy.

YEs I have a very good experiance 4-5 months before with BLS Newyork Staff they are really awsome, there is one guy he is super and helpful and quick . unfortunately I forgot his name ( a Desi guy ) he is such a helpful to resolve any kind of problematic issue. Even thought their website is sometime confusing and Customer service will only provide the policy instructions this GUy helped me outof the box approch. not sure this guy is there right now...But Hat for him and all the staffs

My scenario is.... actually in my daughter's passport the file number is of 15 characters where as while filling up NRI Passport Application form, there it was asking 12 characters (MAX), now I should trim the left 3 characters or right 3 characters.....to proceed ahead. Please advice.

The application instructions from Cox and Kings says that I should fill out lines 9 to 29 MANUALLY with a pen. Is there any specific reason for it. I used FOXIT pdf reader and filled it out with blue font on computer. Is there any reason why they want it handwritten. There are a lot of chances of mistakes happening if handwritten, especially considering the fact that some blanks are so small that not all of the information can be squeezed in without making it unreadably small when written. With computer, we can use small font and will still remain clearly readable.
Will the processing be delayed if we type it in instead of handwriting. This might appear to be a non issue to others but with Indian Goverment stuff, you never know.

The Indian passport has File number with 15 characters (2alpha + 13numeric).
The Online passport application asks for the File no. with max 12 characters.
